Orch Pipeline

The shared engine behind the orch-add-feature, orch-fix-defect, orch-refine-code, orch-change-feature, and orch-build-mvp skills. It's a gated pipeline that classifies request size (trivial to large), then runs research, planning, TDD implementation, code review, and commits in the right order. Two human gates keep you in control: one after planning shows you the task list, another before commit shows you the diff. The operation wrappers tell this engine which phases to run and what first move to make. You won't invoke it directly, but if you're adding a new operation to the family or tweaking how the gates work, this is the reference.
