Well-Being Check

Connects Claude to three structured mental health self-assessments: GAD-7 for anxiety, PHQ-9 for depression, and WHO-5 for general well-being. Exposes a single read-only tool, mental-health-get-test, that loads the requested screening into an interactive widget. Reach for this when you want users to complete a validated questionnaire inside the conversation instead of answering ad-hoc symptom questions. Results stay local and aren't sent anywhere. No authentication required in the current build. Runs as a hosted remote server over streamable-http, so there's nothing to deploy. Informational screenings only, not clinical diagnosis or crisis response.
